Most of my work has been in products where a design mistake has operational consequences: a wrong default puts a wrong menu live across a dozen sales channels, a badly ordered answer costs someone medical time.

That shapes how I work. I'd rather borrow a convention than invent one, and I treat accessibility as infrastructure — fixed at the token level, not screen by screen.
